Tl;dr: The dentists and staff at this office seem great, but the owner of the practice has a state disciplinary history of fraud, negligence, incompetence from at least 2011-2016, and it is unclear as to whether the place is even open anymore. I hope they go back to being Rotunda Dental or become some other practice.
Full version with documents from the state: Dr Bullard and his staff seemed great and accommodating on my visit. Unfortunately, I'm not sure if this company is still even in business. I can't get a hold of them by any means, they return no messages or emails, don't keep tele-health appointments, and their Oakland office is boarded and locked. I am unable to complete the treatment course I set out for. I looked up the licenses of all of the practitioners on the California Department of Consumer Affairs website (feel free to do it yourself for literally anyone who needs to hold any kind of state license) and found the following about the owner of the practice:

There is more, including multiple public documents from the California Attorney General's office, that detail a history of fraud, negligence, and incompetence beginning in 2011, and carrying on until at least 2016. According to the documents, the Dental Board of California was actually trying to revoke Dr Moheb's license, but he landed on probation instead. This is all searchable public info on the DCA website.
